Standards and Expectations Plan by Touchpoint – Nurse (RN)

Day of Diagnostics – Intake & Clinical Anchor
Purpose: Establish a sense of calm, competence, and continuity for the member throughout the day.
Expectations:

  • Greet the member warmly during clinical intake and explain what to expect from each test.
  • Review pre-visit vitals, medical history, and any red flags from the EMR.
  • Provide reassurance throughout procedures, answering clinical questions with empathy and clarity.
  • Act as the liaison between the member and the care team, ensuring all concerns are addressed.
  • Coordinate seamlessly with radiology technicians, Physicians, and Medical Assistants.

Follow-Up: Document the day’s interactions, ensure physician notes are updated, and communicate any emotional or physical concerns noticed during the day to the Care Coordinator and Nurse Practitioner. Be proactive in initiating any follow-up orders or clarifications needed before the post-diagnostic meeting.

Member Concerns or Questions – Ongoing Clinical Support
Purpose: Maintain continuity of care and emotional safety between touchpoints.
Expectations:

  • Respond to member-reported concerns, whether clinical or emotional, in a timely and compassionate manner.
  • Coordinate with the Care Coordinator or Nurse Practitioner to ensure that no issue lingers or goes unresolved.
  • Track and escalate side effects, new symptoms, or compliance challenges to the appropriate team members.

Follow-Up: Record interactions in the EMR and ensure continuity by looping in the Physician and Health Coach as needed. Maintain clinical integrity and protect member confidence through transparency and accountability.

Testing & Procedures – Clinical Execution
Purpose: Deliver precise, high-quality testing while maintaining member comfort and dignity.
Expectations:

  • Clearly explain the purpose and process of any procedure.
  • Execute all tests with calm professionalism and exceptional attention to detail.
  • Minimize member discomfort and ensure emotional support throughout the experience.

Follow-up: Confirm that test results are routed to the correct provider and accurately logged in the system. Communicate abnormal results promptly to the Physician or NP. Note any emotional or physical reactions in the chart to inform future care plans.


Support During Therapeutic Interventions – Continuity & Observation
Purpose: Serve as the steady presence that ensures safety and understanding during clinical treatments.
Expectations:

  • Monitor vitals and emotional state during interventions.
  • Educate members on aftercare and what to expect post-treatment.
  • Partner with the Health Coach to reinforce adherence where needed.

Follow-Up: Complete post-treatment documentation, alert the team to any necessary follow-ups, and ensure the member has clear instructions and a sense of confidence moving forward.


Core Nursing Standards Across All Touchpoints

Preparation Standards:

  • Review each member’s chart 24 hours before interaction.
  • Confirm all clinical documentation is current and diagnostic orders are complete.
  • Check for alerts, emotional notes, or comments from physicians & the care team. 

Delivery Standards:

  • Approach every member with empathy, professionalism, and attentiveness.
  • Make eye contact, use the member’s name, and explain procedures step by step.
  • Maintain clinical excellence and uphold all health and safety protocols.

Follow-Up Standards:

  • Complete charting within 24 hours.
  • Proactively communicate with the care team when issues arise.
  • Reinforce the member's trust by showing reliability, kindness, and consistency in every task.
